the preparation of the original book, I had to enlist the help of many people,
and in every case I was gratified and encouraged by the enthusiastic and helpful
response I received to my enquiries. Without this invaluable assistance my
task would have been much more difficult, if not impossible. Although there
are too many people involved for me to mention them all by name, I would particularly
